Chronic Disease Management

A chronic medical condition is one that has been (or is likely to be) present for six months or longer and can include conditions such as:

  • arthritis
  • asthma
  • cancer
  • cardiovascular disease
  • diabetes
  • mental health conditions (anxiety, depression etc.)
  • musculoskeletal conditions
  • obesity
  • stroke

If you suffer from a chronic medical condition that will require a multidisciplinary, team-based, care approach from a general practitioner and at least two other health or care providers, you may be eligible for Chronic Disease Management (CDM) - formerly Enhanced Primary Care (EPC).

If our general practitioners determine that a Chronic Disease Management Plan is appropriate, they will work with you and other allied health professionals to coordinate the structured approach that your condition requires.

Patients who have a chronic medical condition that is being managed under a Chronic Disease Management Plan are eligible for Medicare rebates on certain allied health services on referral from their general practitioner.
